چکیده

Is there a place for monotherapy? Yes, there is a place for monotherapy. According to the Seventh report of the Joint National Committee in 2003 (1) and the European guidelines from 2007 (2) and 2009 (3), the first step in hypertension control is changing lifestyle before initiating medical treatment. According to the baseline blood pressure and the presence or absence of target organ damage and risk factors, it appears reasonable to initiate therapy either with a low dose of a single agent or with a low-dose combination of two agents. If low-dose monotherapy is chosen and blood pressure control is not achieved, the next step is to switch to a low dose of a different agent, or to increase the previous agent to full dose. Combination therapy may be used according to the circumstances (Figure 1) if target blood pressure values are not achieved: systolic blood pressure < 140 mm Hg and diastolic blood pressure < 90 mm Hg in adults and <130/80 in special populations (patients with diabetes, equivalent diseases or chronic renal diseases). Non-pharmacological measures may suffice to normalize blood pressure in patients with grade 1 (mild) hypertension. Grade 2 hypertension may

Addition of sildenafil to bosentan monotherapy in pulmonary arterial hypertension.

Combination therapy has been recommended for the treatment of pulmonary arterial hypertension (PAH). However, there is scant information on combination therapy after failure of monotherapy, particularly in patients with scleroderma-associated PAH (PAH-SSD). From a group of 82 consecutive patients with PAH who received initial bosentan monotherapy, a total of 13 idiopathic PAH (IPAH) and 12 PAH-...

Survival with sildenafil and inhaled iloprost in a cohort with pulmonary hypertension: an observational study.

BACKGROUND Combination therapy is frequently used to treat patients with pulmonary hypertension but few studies have compared treatment regimens. This study examined the long-term effect of different combination regimens of inhaled iloprost and oral sildenafil on survival and disease progression. METHODS This was a retrospective study of patients in the Giessen Pulmonary Hypertension Registry...

Monotherapy versus dual therapy for the initial treatment of hypertension (PATHWAY-1): a randomised double-blind controlled trial

INTRODUCTION Previous studies have suggested that more intensive initial therapy for hypertension results in better long-term blood pressure (BP) control. We test this hypothesis comparing initial monotherapy with dual therapy in the management of essential hypertension. METHODS AND ANALYSIS The study is a prospective, multicentre, double-blind, active-controlled trial in patients with essent...

Background and Objectives: The Cox model is one of the methods used in survival data; however, the use of hierarchical data, such as the data of this study, violates the assumption of independence, the the Cox model cannot be used  assuming independence of observations. One of the important methods for analyzing survival hierarchy data is the use of the multilevel Cox model. In this method, in ...

Initial Use of Ambrisentan plus Tadalafil in Pulmonary Arterial Hypertension.

BACKGROUND Data on the effect of initial combination therapy with ambrisentan and tadalafil on long-term outcomes in patients with pulmonary arterial hypertension are scarce. METHODS In this event-driven, double-blind study, we randomly assigned, in a 2:1:1 ratio, participants with World Health Organization functional class II or III symptoms of pulmonary arterial hypertension who had not pre...
